Learn why you should see a prosthodontist

A Prosthodontist is a dentist who has completed two or three years of further dental education, following graduation from dental school, at a program accredited by the American Dental Association (ADA). This program focuses on the treatment of teeth that are damaged, or missing, (due to decay, wear, trauma, absent, etc.), and the correction of a displeasing smile. They use all forms of tooth restoration and replacement.

Having the expert eye of a specialist diagnosing the condition of your mouth will ensure that no necessary treatment will go unhandled and that the restoration of your mouth will be achieved to the highest possible standard of care, leading to a more natural and longer lasting result.

The mouth, like any other organ of the body, is made up of various parts and each part functions with the other so that the mouth can do the job it is meant to do in the most efficient manner.

Your front teeth are the most obvious part of your mouth because of your smile — one of the first things another person uses to judge you. But, in addition to your smile, the front teeth are shaped to cut into food to make manageable portions for you to chew and also to help make certain sounds of speech. So improper position of your front teeth will not only affect your smile but may also cause difficulty biting into food and problems with speech.

Your back teeth, with the help of your jaw muscles, provide a broad surface on which you can chew your food so that it is more manageable for your digestive system. The muscles of your jaws provide the power needed to chew your food. Missing or badly aligned back teeth i.e. a bad bite, while not visible or painful, causes excess pressure on the remaining teeth and places a greater strain on the muscles of your jaws. This inevitably leads to some problems such as teeth fracturing, fillings breaking and tooth wear.

Unfortunately, just like other organs, if we do not feel pain, we tend to assume that everything is just fine. We all know that that is not the case. For example, we can't see the arteries in the heart clogging up and often we are not aware that our blood pressure is high and we don't have chest pain, so we don't know that our heart is not functioning as it should even though one of its parts is not well. This can have fatal consequences.

While it is highly unlikely that anyone is going to die because they have a missing tooth, damaged or missing teeth cause problems which we all too often are unaware of.

Case History 1

The first is a patient who came in with an old upper full denture and a bridge on the lower teeth. She was very anxious about getting any dental work done due to prior experiences that were not great, and that had kept her from seeing a dentist. She was not at all happy about the appearance of her teeth or smile and she had been starting to have trouble keeping her upper denture in place.

As you can see the teeth are crooked and the denture has been repaired several times due to teeth falling off the denture because only the front teeth hit. That is where she does all her chewing, which inevitably causes the teeth to break. Within a few weeks, a new upper full denture and a lower bridge with a removable partial denture was made providing this patient with the back teeth she needed to chew effectively and give stability to her upper denture. With the teeth in the correct alignment, not only is her smile more pleasing but also longer lasting and more natural.

Case History 2

Some patients do not lose their teeth but instead can find that their own teeth have changed in size, shape and color over a period of time, not only due to aging but also through habits such as grinding one's teeth, or due to other conditions which cause the teeth to wear or dissolve away prematurely. The proper diagnosis as to the cause of the tooth surface loss is of great importance as this will affect the the decision on the design and choice of materials used to restore the teeth.

This patient had been losing the edges of her teeth due to a grinding habit that was made worse by medical condition which caused erosion of the back teeth. This lead to difficulty eating. It was important to establish that these things had caused the tooth surface loss and discoloration so that the final choice of materials would be such that they would be most appropriate for both esthetics and longevity.

The patient was embarrassed to smile and very conscious of how her teeth looked. There was no option other than to place a dental restoration on all her teeth, to restore the bite back to its original position and provide the correct tooth alignment. To ensure that the final teeth would be comfortable and function well, laboratory made provisional (temporary) teeth are made which serve to guide both the shape and size of the teeth and also the bite. This is a crucial step in the restoration of complex cases.

Once the patient was comfortable with her provisional restorations, the final crowns and veneers are made. The results are below. Now the patient loves her smile and has her teeth back to their former size, shape and color.

Case History 3

This patient came in with an upper removable partial denture which had been made to patch the places where prior bridgework had failed. This piecemeal work lead to the crooked and mismatched appearance of her teeth. She did not relish coming to the dentist and getting work done and would reluctantly see her general dentist for the odd filling and crown but never to get her teeth fixed. Her daughter met Dr. Master and she brought her mother to see him.

The patient was not in any pain and did not know what could be done to fix the problems she knew she had. Dr Master approached the problem in a step by step manner, first showing the patient exactly where the upper front teeth SHOULD be placed and then from that basic step, showing her how the other teeth could then be made. The patient was reluctant to lose the bridge she had that replaced her front teeth because although it did not look good, she was sure that it would be better than a full denture. So Dr Master kept some of the roots of the teeth that had once held the bridge but could not support another one, and used them to support and hold an upper denture. This type of denture is called an OVERDENTURE, because it goes OVER the roots of the teeth. These roots help preserve the bone and provide support and, as you can see from the pictures below, can be made with a device that helps keep the denture in place (retain is the technical word!). Similarly, a holding device, called a “precision attachment” was placed on the crowns that held the lower partial denture, ensuring great stability.

Once these restorations were made, the patient commented that the new teeth looked just like her teeth did in her 20's. Moreover her fears of having problems with a full upper denture were gone. In fact she had to work to get the dentures out!
